U Opću bolnicu Zadar se sa specijalizacije iz plastično-rekonstruktivne kirurgije vratio dr. Domagoj Župan. Odmah po njegovu dolasku u Službu za kirurgiju, ponovno je, poslije dugih 15 godina, pokrenut program rekonstrukcije dojke nakon operacije zloćudnog tumora. Prva je zahvatu podvrgnuta jedna naša sugrađanka kojoj je odstranjena cijela dojka. Kako bi se ona iznova oblikovala i kako bi se postigla simetrija dojki, odabran je odgovarajući implantat. Kada se tim liječnika uvjerio da je postoperacijski tijek prošao bez komplikacija, ovu su veliku vijest odlučili podijeliti sa svima nama. Desetljeće i pol onkološke pacijentice s našeg područja svoju želju za rekonstrukcijom dojki mogle su ostvariti na jednoj od klinika za plastično-rekonstruktivnu kirurgiju u Republici Hrvatskoj, a sada im je ona opet dostupna u našem gradu.

- Smatra se da je rekonstrukcija dojki dio samog liječenja karcinoma. Sve žene kojima je potreban radikalan zahvat imaju pravo na rekonstrukciju, neovisno o njihovoj dobi. Dok sama bradavica nije oblikovana, liječenje nije završeno. Od velikog je to značaja za psihološko zdravlje žene, ali i za njen cjelokupan oporavak od onkološkog liječenja, kaže nam dr.Župan.

Dva su načina rekonstrukcije dojke. Prvi podrazumijeva korištenje umjetnog materijala, odnosno silikonskog implantata. Kada se dojka odstrani, sačuva se njena koža, a ukoliko onkološki nalaz dopušta, sačuva se i bradavica. Implantat se koristi kako bi se nadoknadio izgubljeni volumen dojke.

Druga opcija jest rekonstrukcija autolognim tkivom. Dakle, uzima se tkivo s trbuha ili leđa pacijentice što ostavlja morbiditet donorske regije s koje je tkivo uzeto. U praksi je stoga češće korištena rekonstrukcija implantatom.

Operaciju je najbolje izvesti odmah, dakle istovremeno s mastektomijom jer sama rekonstrukcija ne utječe na daljnji tijek onkološkog liječenja. No, ako iz bilo kojeg razloga primarna rekonstrukcija nije moguća ili joj se žena ne želi podvrgnuti, postoji i opcija one sekundarne.

Kod velikog broja pacijentica koje boluju od karcinoma dojke rade se tzv.poštedne operacije, odnosno odstrani se samo bolesni dio dojke. Tom prilikom ipak nastaje defekt, također estetski neprihvatljiv. 

- I tu je moguća rekonstrukcija. S trbuha ili sa stražnjice pacijentice se uzme masno tkivo koje se koristi za rekonstrukciju tog defekta. Uskoro ćemo moći raditi i taj zahvat, istaknuo je dr.Župan.

Dok razgovaramo s dr.Županom, kraj njega sjedi i sve sa zadovoljstvom prati izv.prof.dr.sc. Ivan Bačić, dr.med, pročelnik Službe za kirurgiju. 

- Dolazak dr.Župana donio je jednu pozitivnu energiju među nas. On je sad punopravni član našeg tima za liječenje bolesti dojke i naša velika nada koja će educirati mlađe kirurge. Evo, već imamo specijalizanticu koja će raditi s dr.Županom i koja će jednog dana preuzeti svoj dio bolesnika. S ponosom sada mogu kazati kako pacijentice u OB Zadar mogu dobiti kompletnu kiruršku skrb; onkološko liječenje i plastično – rekonstruktivno liječenje. Kada stigne linearni akcelerator pokrit ćemo baš sve, od ranog screeninga do rekonstrukcije. Očekujemo kako će u našu ustanovu dolaziti pacijentice ne samo sa zadarskog područja, već i iz Like i južnijih dalmatinskih županija, zaključuje dr.Bačić.

Nažalost, pacijentica je više nego li prije pandemije, upozorava prvi čovjek zadarske Kirurgije. Godišnje se otkrije oko 120 karcinoma dojke na našem području, a obolijevaju žene u dobi od 30, pa sve do 90 godina.

- Borimo se s karcinomom, već godinama. No, naš tim kirurga sve je jači. Zadarska kirurgija ne miruje, stalno nastojimo svjetske trendove u kirurgiji primijeniti u našoj ustanovi, za dobrobit naših pacijenata, zaključuje dr. Bačić.

U Opću bolnicu Zadar se sa specijalizacije iz plastično-rekonstruktivne kirurgije vratio dr. Domagoj Župan. Odmah po njegovu dolasku u Službu za kirurgiju, ponovno je, poslije dugih 15 godina, pokrenut program rekonstrukcije dojke nakon operacije zloćudnog tumora. Prva je zahvatu podvrgnuta jedna naša sugrađanka kojoj je odstranjena cijela dojka. Kako bi se ona iznova oblikovala i kako bi se postigla simetrija dojki, odabran je odgovarajući implantat. Kada se tim liječnika uvjerio da je postoperacijski tijek prošao bez komplikacija, ovu su veliku vijest odlučili podijeliti sa svima nama. Desetljeće i pol onkološke pacijentice s našeg područja svoju želju za rekonstrukcijom dojki mogle su ostvariti na jednoj od klinika za plastično-rekonstruktivnu kirurgiju u Republici Hrvatskoj, a sada im je ona opet dostupna u našem gradu.

- Smatra se da je rekonstrukcija dojki dio samog liječenja karcinoma. Sve žene kojima je potreban radikalan zahvat imaju pravo na rekonstrukciju, neovisno o njihovoj dobi. Dok sama bradavica nije oblikovana, liječenje nije završeno. Od velikog je to značaja za psihološko zdravlje žene, ali i za njen cjelokupan oporavak od onkološkog liječenja, kaže nam dr.Župan.

Dva su načina rekonstrukcije dojke. Prvi podrazumijeva korištenje umjetnog materijala, odnosno silikonskog implantata. Kada se dojka odstrani, sačuva se njena koža, a ukoliko onkološki nalaz dopušta, sačuva se i bradavica. Implantat se koristi kako bi se nadoknadio izgubljeni volumen dojke.

Druga opcija jest rekonstrukcija autolognim tkivom. Dakle, uzima se tkivo s trbuha ili leđa pacijentice što ostavlja morbiditet donorske regije s koje je tkivo uzeto. U praksi je stoga češće korištena rekonstrukcija implantatom.

Operaciju je najbolje izvesti odmah, dakle istovremeno s mastektomijom jer sama rekonstrukcija ne utječe na daljnji tijek onkološkog liječenja. No, ako iz bilo kojeg razloga primarna rekonstrukcija nije moguća ili joj se žena ne želi podvrgnuti, postoji i opcija one sekundarne.

Kod velikog broja pacijentica koje boluju od karcinoma dojke rade se tzv.poštedne operacije, odnosno odstrani se samo bolesni dio dojke. Tom prilikom ipak nastaje defekt, također estetski neprihvatljiv. 

- I tu je moguća rekonstrukcija. S trbuha ili sa stražnjice pacijentice se uzme masno tkivo koje se koristi za rekonstrukciju tog defekta. Uskoro ćemo moći raditi i taj zahvat, istaknuo je dr.Župan.

Dok razgovaramo s dr.Županom, kraj njega sjedi i sve sa zadovoljstvom prati izv.prof.dr.sc. Ivan Bačić, dr.med, pročelnik Službe za kirurgiju. 

- Dolazak dr.Župana donio je jednu pozitivnu energiju među nas. On je sad punopravni član našeg tima za liječenje bolesti dojke i naša velika nada koja će educirati mlađe kirurge. Evo, već imamo specijalizanticu koja će raditi s dr.Županom i koja će jednog dana preuzeti svoj dio bolesnika. S ponosom sada mogu kazati kako pacijentice u OB Zadar mogu dobiti kompletnu kiruršku skrb; onkološko liječenje i plastično – rekonstruktivno liječenje. Kada stigne linearni akcelerator pokrit ćemo baš sve, od ranog screeninga do rekonstrukcije. Očekujemo kako će u našu ustanovu dolaziti pacijentice ne samo sa zadarskog područja, već i iz Like i južnijih dalmatinskih županija, zaključuje dr.Bačić.

Nažalost, pacijentica je više nego li prije pandemije, upozorava prvi čovjek zadarske Kirurgije. Godišnje se otkrije oko 120 karcinoma dojke na našem području, a obolijevaju žene u dobi od 30, pa sve do 90 godina.

- Borimo se s karcinomom, već godinama. No, naš tim kirurga sve je jači. Zadarska kirurgija ne miruje, stalno nastojimo svjetske trendove u kirurgiji primijeniti u našoj ustanovi, za dobrobit naših pacijenata, zaključuje dr. Bačić.

U zadarskoj Općoj bolnici nakon 15 godina ponovno pokrenut program rekonstrukcije dojke nakon operacije zloćudnog tumora

U Opću bolnicu Zadar se sa specijalizacije iz plastično-rekonstruktivne kirurgije vratio dr. Domagoj Župan. Odmah po njegovu dolasku u Službu za kirurgiju, ponovno je, poslije dugih 15 godina, pokrenut program rekonstrukcije dojke nakon operacije zloćudnog tumora. Prva je zahvatu podvrgnuta jedna naša sugrađanka kojoj je odstranjena cijela dojka. Kako bi se ona iznova oblikovala i kako bi se postigla simetrija dojki, odabran je odgovarajući implantat. Kada se tim liječnika uvjerio da je postoperacijski tijek prošao bez komplikacija, ovu su veliku vijest odlučili podijeliti sa svima nama. Desetljeće i pol onkološke pacijentice s našeg područja svoju želju za rekonstrukcijom dojki mogle su ostvariti na jednoj od klinika za plastično-rekonstruktivnu kirurgiju u Republici Hrvatskoj, a sada im je ona opet dostupna u našem gradu.

- Smatra se da je rekonstrukcija dojki dio samog liječenja karcinoma. Sve žene kojima je potreban radikalan zahvat imaju pravo na rekonstrukciju, neovisno o njihovoj dobi.
