O koronavirusu je napisano više znanstvenih studija nego o bilo kojoj drugoj bolesti na svijetu, no još uvijek o COVID-u 19 ne znamo dovoljno. Od početka pandemije postoje i kontroverzna stajališta o zaraznosti asimptomatskih bolesnika. Tako se među liječnicima i znanstvenicima vrlo često može čuti mišljenje da se osobe koje nemaju simptome bolesti, a pozitivne su na test COVID-19, smatraju manje zaraznima za okolinu nego oni koji imaju izražene simptome bolesti.

Ima tu i logike budući ne kišu i ne sline, virus ne šire velikom brzinom dalje kao oni sa simptomima.

Međutim sa zaraznim bolestima situacija nije baš tako jednostavna, a pogotovo ne s Covidom koji je složena slagalica s mnogo dijelova, različitih boja i oblika.

Svaka bolest se definira s prisutnošću simptoma, no to je uvijek upitno sa zaraznim bolestima. Primjerice kod HIV-a, također zarazne bolesti, virus se može nositi i deset godina, a da osoba nema simptome, a cijelo vrijeme je zarazna za druge. Smatra se da kod COVID-a oko 40 posto bolesnika nema simptome, a najnovija studija upravo je pokazala da osobe koje su zaražene koronavirusom, a nemaju nikakve simptome, imaju čak i veće virusno opterećenje od simptomatskih pacijenata oboljelih od COVID-19.

To praktički znači da su asimptomatski pacijenti čak zarazniji od simptomatskih.

Da bi došli do svojih otkrića, znanstvenici su u studiji, koja je objavljena u znanstvenom časopisu Infection, analizirali briseve bolničkih pacijenata iz nosa, ždrijela, usne šupljine, sline, urina i rektuma.

Proučavali su virusna opterećenja na šest različitih tipova uzoraka uzetih od COVID-19 pacijenata različite dobi i različite kliničke slike kako bi utvrdili odnos između tijeka bolesti i virusnog opterećenja.

U istraživanje su uključeni pacijenti koji su hospitalizirani između 1. i 30. svibnja 2020., u Klinici za infektivne bolesti u Ankari. Pacijenti koje nisu pratili najmanje jedan mjesec isključeni su iz studije. Analizirano je 360 uzoraka od 60 pacijenata. Dvadeset i pet posto tih pacijenata nije pokazivalo nikakve simptome bolesti. Demografske značajke poput starosti, spola, mjesta prebivališta, simptoma i vremena početka i popratnih bolesti, tjelesnog pregleda, vrućice i vitalnih znakova praćene su grafikonima.

Analizirani su laboratorijski testovi kao što su kompletna krvna slika, napravljena kemijska analiza krvi, parametri koagulacije, reaktanti akutne faze, RTG i CT prsnog koša, a pacijenti su klasificirani u tri kategorije – oni s blagom bolešću, upalom pluća te oni s teškim respiratornim sindromom koji su liječeni na klinici.

Analizom svih tih parametara zaključeno je kako je virusno opterećenje kod asimptomatskih pacijenata veće nego kod simptomatskih pacijenata. Isto tako, otkriveno je da virusno opterećenje ima negativan trend s višom dobi pacijenata te da je značajno smanjeno što je bolest teža. Čini se da su faktori povezani s loših ishodom liječenja, poput niskog broja limfocita u značajnoj korelaciji s malim virusnim opterećenjem SARS-CoV-2.

Virusna opterećenja pacijenata s najmanje jednim komorbiditetom ( hipertenzijom, dijabetesom, malignom bolesti, kroničnom plućnom bolesti) značajno su niža od pacijenata bez komorbiditeta. Nadalje, utvrđeno je da je kod pacijenata s najmanje jednim komorbiditetom znatno vjerojatnije da će imati teže simptome.  

I današnji novi rekord broja umrlih u Hrvatskoj, dodatni je apel svima onima, u prvom redu mladim i zdravim ljudima, koji su najučestaliji asimptomatski širitelji zaraze.

Poštujete epidemiološke mjere, nosite maske i držite distancu, jer iako nemate nikakvih teškoća, možete biti izvor životne opasnosti za druge, pogotovo za svoje starije ukućane i druge kronične bolesnike.

Od početka pandemije postoje i kontroverzna stajališta o zaraznosti asimptomatskih bolesnika. Tako se među liječnicima i znanstvenicima vrlo često može čuti mišljenje da se osobe koje nemaju simptome bolesti, a pozitivne su na test COVID-19, smatraju manje zaraznima za okolinu nego oni koji imaju izražene simptome bolesti.

Ima tu i logike budući ne kišu i ne sline, virus ne šire velikom brzinom dalje kao oni sa simptomima.

Međutim sa zaraznim bolestima situacija nije baš tako jednostavna, a pogotovo ne s Covidom koji je složena slagalica s mnogo dijelova, različitih boja i oblika.

Svaka bolest se definira s prisutnošću simptoma, no to je uvijek upitno sa zaraznim bolestima. Primjerice kod HIV-a, također zarazne bolesti, virus se može nositi i deset godina, a da osoba nema simptome, a cijelo vrijeme je zarazna za druge. Smatra se da kod COVID-a oko 40 posto bolesnika nema simptome, a najnovija studija upravo je pokazala da osobe koje su zaražene koronavirusom, a nemaju nikakve simptome, imaju čak i veće virusno opterećenje od simptomatskih pacijenata oboljelih od COVID-19.

To praktički znači da su asimptomatski pacijenti čak zarazniji od simptomatskih.

Da bi došli do svojih otkrića, znanstvenici su u studiji, koja je objavljena u znanstvenom časopisu Infection, analizirali briseve bolničkih pacijenata iz nosa, ždrijela, usne šupljine, sline, urina i rektuma.

Proučavali su virusna opterećenja na šest različitih tipova uzoraka uzetih od COVID-19 pacijenata različite dobi i različite kliničke slike kako bi utvrdili odnos između tijeka bolesti i virusnog opterećenja.

U istraživanje su uključeni pacijenti koji su hospitalizirani između 1. i 30. svibnja 2020., u Klinici za infektivne bolesti u Ankari. Pacijenti koje nisu pratili najmanje jedan mjesec isključeni su iz studije. Analizirano je 360 uzoraka od 60 pacijenata. Dvadeset i pet posto tih pacijenata nije pokazivalo nikakve simptome bolesti. Demografske značajke poput starosti, spola, mjesta prebivališta, simptoma i vremena početka i popratnih bolesti, tjelesnog pregleda, vrućice i vitalnih znakova praćene su grafikonima.

Analizirani su laboratorijski testovi kao što su kompletna krvna slika, napravljena kemijska analiza krvi, parametri koagulacije, reaktanti akutne faze, RTG i CT prsnog koša, a pacijenti su klasificirani u tri kategorije – oni s blagom bolešću, upalom pluća te oni s teškim respiratornim sindromom koji su liječeni na klinici.

Analizom svih tih parametara zaključeno je kako je virusno opterećenje kod asimptomatskih pacijenata veće nego kod simptomatskih pacijenata. Isto tako, otkriveno je da virusno opterećenje ima negativan trend s višom dobi pacijenata te da je značajno smanjeno što je bolest teža. Čini se da su faktori povezani s loših ishodom liječenja, poput niskog broja limfocita u značajnoj korelaciji s malim virusnim opterećenjem SARS-CoV-2.

Virusna opterećenja pacijenata s najmanje jednim komorbiditetom ( hipertenzijom, dijabetesom, malignom bolesti, kroničnom plućnom bolesti) značajno su niža od pacijenata bez komorbiditeta. Nadalje, utvrđeno je da je kod pacijenata s najmanje jednim komorbiditetom znatno vjerojatnije da će imati teže simptome.  

I današnji novi rekord broja umrlih u Hrvatskoj, dodatni je apel svima onima, u prvom redu mladim i zdravim ljudima, koji su najučestaliji asimptomatski širitelji zaraze.

Poštujete epidemiološke mjere, nosite maske i držite distancu, jer iako nemate nikakvih teškoća, možete biti izvor životne opasnosti za druge, pogotovo za svoje starije ukućane i druge kronične bolesnike.

O koronavirusu je napisano više znanstvenih studija nego o bilo kojoj drugoj bolesti na svijetu, no još uvijek o COVID-u 19 ne znamo dovoljno.
