Uz veću svijest o važnosti prepoznavanja simptoma i liječenja u populaciji te uz lijekove koje imamo na raspolaganju i koji su jako moćni, pojava čira može se vrlo elegantno spriječiti

Dobra vijest kad je riječ o čirevima na želucu i dvanaesniku jest to da ih ima sve manje.

- To je zato što pacijent kad dođe liječniku obiteljske medicine ili gastroenterologu zbog spomenutih dispeptičkih simptoma, većinom odmah dobije terapiju kojom se suprimira lučenje želučane kiseline i očito je da se time jako sprečava nastanak naprednijih oblika gastritisa, tj. pojava čira i svih mogućih komplikacija koje uz čir mogu nastati. U posljednje vrijeme zaista rijetko vidimo pacijente koji nam dođu s klasičnim čirom na želucu koji je prije 20 godina bio jako čest. Sada se vidi da se, uz veću svijest o važnosti prepoznavanja simptoma i liječenja u populaciji te uz lijekove koje imamo na raspolaganju i koji su jako moćni, pojava čira može vrlo elegantno spriječiti - kaže dr. sc. Pave Markoš, gastroenterolog i hepatolog iz KBC-a Zagreb.

Svaki čir na želucu je premaligna promjena

- Čir na želucu uvijek je ozbiljnija stvar od čira na dvanaesniku i svaki čir na želucu je premaligna promjena te se kao takva ne može zanemariti kad se uoči na gastroskopiji. Takav pacijent mora za dva mjeseca doći na kontrolnu gastroskopiju da se vidi je li čir odgovarajuće zacijelio. Kad se nekome gastroskopijom otkrije čir na želucu, odmah mora dobiti terapiju inhibitorima protonske pumpe, odnosno lijekovima kojima se suprimira želučana kiselina, mora mu se tijekom gastroskopije napraviti biopsija da se vidi ima li Helicobacter te mora ići na kontrolnu gastroskopiju za dva do tri mjeseca jer je to period u kojem očekujemo da će čir zacijeljeti. Ako uza svu terapiju čir za dva mjeseca ne zacijeli, to mora pobuditi sumnju da možda nije riječ o čiru nego već o premalignoj promjeni - ističe dr. sc. Markoš, dodajući da su uzrok čireva najčešće Helicobacter i lijekovi iz grupe nesteroidnih protuupalnih lijekova (ketonal, voltareni...), pogotovo u starijoj populaciji koja ih uzima zbog različitih reumatoloških i koštano-zglobnih tegoba.

Noćna bol

Inače, čir kao takav ne mora stvarati simptome. No, ako osoba nakon obroka osjeća jaku težinu, ako počne gubiti tjelesnu težinu i apetit, onda treba posumnjati na čir. Simptomi kompliciranih čireva su mučnina, bolovi, intolerancija hrane, povraćanje, gubitak apetita, bolovi koji se mogu javiti nakon obroka ili natašte, a pogotovo bolovi koji bude osobu tijekom noći.

- Općenito unutar gastroenterologije sve što se događa tijekom noći, ako pacijenta probudi bol ili neki drugi simptom, nije dobar znak jer, u pravilu, ako pacijent nema tešku gastroenterološku bolest, ne bi trebao imati noćne simptome - kaže dr. sc. Markoš, dodajući da se čir može komplicirati i tako da svojim rastom erodira krvnu žilu i da prokrvari. Tada osoba može povraćati krv i imati crnu stolicu, ali to su kasne faze bolesti koje se danas događaju relativno rijetko, kod ljudi koji se ne jave liječniku zbog simptoma ili koji godinama uzimaju voltarene, ketonale i slične lijekove za bolove u kralježnici i onda se kod gastroenterologa pojave kad je čir već u toj uznapredovaloj fazi.

Lijekovi i ulkusna dijeta

- Prvo otkrivamo uzrok čira. Ako je uzrok neki lijek iz skupine NSAR lijekova ili antiagregacijskih ili antikoagulantnih, procjenjujemo je li te lijekove nužno potrebno nastaviti uzimati. Ako ih pacijent uzima jer mu povoljno djeluju u prevenciji moždanog ili srčanog udara, onda se s tim lijekovima mora nastaviti, ali onda uz njih dajemo i lijekove kojima se suprimira lučenje želučane kiseline i štiti želudac. S druge strane, ako nije nužno da pacijent uzima te lijekove koji stvaraju nuspojave na želucu, prekidamo tu terapiju i dajemo neke druge lijekove koji nemaju tako velike nuspojave na želudac. Ako je pak uzrok Helicobacter, moramo ga liječiti i obavezno se svaki čir mora ispratiti kontrolnom gastroskopijom. Uz terapiju, osoba mora biti i na ulkusnoj dijeti, a to je dijeta koja je mediteranskog tipa.

Drugim riječima, trebalo bi izbjegavati preljutu, preprženu i jako začinjenu hranu, paziti da se više jede kuhana hrana, češće bijelo meso, bez prejakih začina te pogotovo bez velike količine masnoće. No, to nije trajno jer ako na kontrolnoj gastroskopiji vidite da je čir zacijelio i da ste izliječili Helicobacter pylori, onda već nakon šest do osam tjedana možete reći osobi da se vrati normalnoj prehrani jer nema smisla da pacijenta stalno držite na dijeti - kaže dr. sc. Markoš, no dodaje da i kad je čir izliječen, treba malo pripaziti na hranu.

- Naime, ako osoba malo pretjera i u dva tjedna se prejede nečeg groznog, od toga sigurno neće nastati čir. Ali ako svaki dan jede svinjetinu, prženu, ljutu i začinjenu hranu, simptomi će se vratiti - kaže dr. sc. Pave Markoš, gastroenterolog i hepatolog iz KBC-a Zagreb.

Uz veću svijest o važnosti prepoznavanja simptoma i liječenja u populaciji te uz lijekove koje imamo na raspolaganju i koji su jako moćni, pojava čira može se vrlo elegantno spriječiti

Dobra vijest kad je riječ o čirevima na želucu i dvanaesniku jest to da ih ima sve manje.

- To je zato što pacijent kad dođe liječniku obiteljske medicine ili gastroenterologu zbog spomenutih dispeptičkih simptoma, većinom odmah dobije terapiju kojom se suprimira lučenje želučane kiseline i očito je da se time jako sprečava nastanak naprednijih oblika gastritisa, tj. pojava čira i svih mogućih komplikacija koje uz čir mogu nastati. U posljednje vrijeme zaista rijetko vidimo pacijente koji nam dođu s klasičnim čirom na želucu koji je prije 20 godina bio jako čest. Sada se vidi da se, uz veću svijest o važnosti prepoznavanja simptoma i liječenja u populaciji te uz lijekove koje imamo na raspolaganju i koji su jako moćni, pojava čira može vrlo elegantno spriječiti - kaže dr. sc. Pave Markoš, gastroenterolog i hepatolog iz KBC-a Zagreb.

Svaki čir na želucu je premaligna promjena

- Čir na želucu uvijek je ozbiljnija stvar od čira na dvanaesniku i svaki čir na želucu je premaligna promjena te se kao takva ne može zanemariti kad se uoči na gastroskopiji. Takav pacijent mora za dva mjeseca doći na kontrolnu gastroskopiju da se vidi je li čir odgovarajuće zacijelio. Kad se nekome gastroskopijom otkrije čir na želucu, odmah mora dobiti terapiju inhibitorima protonske pumpe, odnosno lijekovima kojima se suprimira želučana kiselina, mora mu se tijekom gastroskopije napraviti biopsija da se vidi ima li Helicobacter te mora ići na kontrolnu gastroskopiju za dva do tri mjeseca jer je to period u kojem očekujemo da će čir zacijeljeti. Ako uza svu terapiju čir za dva mjeseca ne zacijeli, to mora pobuditi sumnju da možda nije riječ o čiru nego već o premalignoj promjeni - ističe dr. sc. Markoš, dodajući da su uzrok čireva najčešće Helicobacter i lijekovi iz grupe nesteroidnih protuupalnih lijekova (ketonal, voltareni...), pogotovo u starijoj populaciji koja ih uzima zbog različitih reumatoloških i koštano-zglobnih tegoba.

Noćna bol

Inače, čir kao takav ne mora stvarati simptome. No, ako osoba nakon obroka osjeća jaku težinu, ako počne gubiti tjelesnu težinu i apetit, onda treba posumnjati na čir. Simptomi kompliciranih čireva su mučnina, bolovi, intolerancija hrane, povraćanje, gubitak apetita, bolovi koji se mogu javiti nakon obroka ili natašte, a pogotovo bolovi koji bude osobu tijekom noći.

- Općenito unutar gastroenterologije sve što se događa tijekom noći, ako pacijenta probudi bol ili neki drugi simptom, nije dobar znak jer, u pravilu, ako pacijent nema tešku gastroenterološku bolest, ne bi trebao imati noćne simptome - kaže dr. sc. Markoš, dodajući da se čir može komplicirati i tako da svojim rastom erodira krvnu žilu i da prokrvari. Tada osoba može povraćati krv i imati crnu stolicu, ali to su kasne faze bolesti koje se danas događaju relativno rijetko, kod ljudi koji se ne jave liječniku zbog simptoma ili koji godinama uzimaju voltarene, ketonale i slične lijekove za bolove u kralježnici i onda se kod gastroenterologa pojave kad je čir već u toj uznapredovaloj fazi.

Lijekovi i ulkusna dijeta

- Prvo otkrivamo uzrok čira. Ako je uzrok neki lijek iz skupine NSAR lijekova ili antiagregacijskih ili antikoagulantnih, procjenjujemo je li te lijekove nužno potrebno nastaviti uzimati. Ako ih pacijent uzima jer mu povoljno djeluju u prevenciji moždanog ili srčanog udara, onda se s tim lijekovima mora nastaviti, ali onda uz njih dajemo i lijekove kojima se suprimira lučenje želučane kiseline i štiti želudac. S druge strane, ako nije nužno da pacijent uzima te lijekove koji stvaraju nuspojave na želucu, prekidamo tu terapiju i dajemo neke druge lijekove koji nemaju tako velike nuspojave na želudac. Ako je pak uzrok Helicobacter, moramo ga liječiti i obavezno se svaki čir mora ispratiti kontrolnom gastroskopijom. Uz terapiju, osoba mora biti i na ulkusnoj dijeti, a to je dijeta koja je mediteranskog tipa.

Drugim riječima, trebalo bi izbjegavati preljutu, preprženu i jako začinjenu hranu, paziti da se više jede kuhana hrana, češće bijelo meso, bez prejakih začina te pogotovo bez velike količine masnoće. No, to nije trajno jer ako na kontrolnoj gastroskopiji vidite da je čir zacijelio i da ste izliječili Helicobacter pylori, onda već nakon šest do osam tjedana možete reći osobi da se vrati normalnoj prehrani jer nema smisla da pacijenta stalno držite na dijeti - kaže dr. sc. Markoš, no dodaje da i kad je čir izliječen, treba malo pripaziti na hranu.

- Naime, ako osoba malo pretjera i u dva tjedna se prejede nečeg groznog, od toga sigurno neće nastati čir. Ali ako svaki dan jede svinjetinu, prženu, ljutu i začinjenu hranu, simptomi će se vratiti - kaže dr. sc. Pave Markoš, gastroenterolog i hepatolog iz KBC-a Zagreb.
