Jedan od najvećih roditeljskih strahova je onaj da im dijete, dok je izvan dosega njihova vidna polja, ne udahne (proguta) neko strano tijelo koje bi moglo doći do dušnika ili bronha i na taj način proizvesti velike tegobe.
- Mala nepažnja može biti vrlo opasna, životno ugrožavajuća za dijete kojemu se nešto takvo dogodi - kazuje nam doc. dr. sc. Mirko Kontić, specijalist otorinolaringologije i subspecijalist plastične kirurgije glave i vrata sa Klinike za bolesti uha, grla i nosa s kirurgijom glave i vrata KBC-a Split.
Ako se nešto takvo dogodi, nad djetetom je potrebno obaviti ventilacijsku rigidnu traheobronhoskopiju (vađenje krutim tubusima stranih tijela iz donjih dišnih putova, dušnika ili bronha).
- Takvog pacijenta anesteziolog uspava i nakon toga mi pristupamo vađenju stranog tijela. Za to upotrebljavamo optičke hvataljke, uz čiju pomoć vidimo cijeli tijek zahvata i možemo ga “pod kontrolom oka” kontrolirati svaku sekundu i u slučaju poteškoća reagirati na vrijeme.
Pao na leđa i udahnuo ga
Ovi optički instrumenti (vrsta rigidnih bronhoskopa) umnogome su nam pomogli u radu, odnosno to je bila svojevrsna revolucija kada smo ih nabavili za bolnicu, govori dr. Kontić.
S obzirom na to da KBC Split ima optičke hvataljke, ovakvi zahvati koji spašavaju živote djece u pravilu ne traju duže od 15 do 20 minuta.
- Međutim, ima i izuzetaka. Tako smo nedavno imali zahvat na djetetu starom tek 19 mjeseci. Ono je dovedeno iz jednog grada srednje Dalmacije gdje je s roditeljima bilo na ljetovanju.

U jednom je trenutku dijete, tko zna kako, uzelo šest komadića kikirikija i stavilo ih u usta. Budući da je potom palo na leđa, udahnulo je taj kikiriki koji mu je ušao u bronhe. Taj zahvat je trajao sat i pol i bio je vrlo težak, jer što je dijete manje i sam zahvat je teži - pojašnjava dr. Kontić.
Roditelji često puta misle da su tegobe izazvane kikirikijima, bombonima... završene nakon što se dijete prestane gušiti ili kašljati.
Tri faze
- Postoje tri faze. Prva faza je kada se dijete guši ili kašljuca nakon što je strano tijelo ušlo u dušnik ili bronhe. Ta faza traje od 15 do 20 minuta i nakon toga prestaje.
Druga faza je faza smirivanja stanja, kada roditelji pomisle da se dijete samo “zagrcnulo”. Posljednja faza je faza komplikacija, kada strano tijelo ostaje u organizmu i od njega može nastati upalni proces, a na kraju može doći i do smrtnog ishoda - upozorava dr. Kontić.
U splitskom KBC-u ovaj postupak vađenja stranih tijela iz dušnika ili bronha obavlja se, prema riječima našeg sugovornika, već 40-ak godina.
Sve se prekida radi toga
- Obavljali su ga prim. dr. Duje Barač, dr. Mladen Puharić, dr. Šimunić i prof. dr. Ivo Glunčić, no oni su tada radili s puno primitivnijim sredstvima nego mi danas, kada imamo optičke hvataljke uz pomoć kojih vidimo cijeli proces vađenja.

Sada osim mene ove postupke radi cijeli tim od nekoliko ljudi s klinike i već educiramo i mlađe kolege koji će nas jednom naslijediti. Vrlo važan dio našeg tima su i anesteziolozi, anesteziološki tehničari i naše sestre instrumentarke.
Ovakvi zahvati su uvijek hitnoća i kada imamo neki takav zahvat, odmah prekidamo s operativnim zahvatom koji je u tijeku kako bismo mogli obaviti ventilacijsku rigidnu traheobronhoskopiju - govori dr. Kontić.
20-30 slučajeva godišnje
Godišnje se u splitskom KBC-u obavi između 20 i 30 ovakvih zahvata, a KBC Split za ove zahvate pokriva područje od Dubrovnika do Zadra i veliki dio susjedne BiH.
- Djeca najčešće aspiriraju (udahnu) kikiriki, lješnjak, badem, mrkvu, kukuruz, komadiće mesa, gumene bombone, kamenčiće s plaže, plastične ili metalne igračke, a nedavno smo jednom odraslijem djetetu izvadili kapicu od puntine.

No, taj je postupak obavljen za nekih 20-ak minuta, jer kao što sam istaknuo, lakše ga je raditi na većem nego na manjem djetetu - zaključio je doc. dr. sc. Mirko Kontić.
Stoga roditelji, dobro pazite i maksimalno brzo reagirajte ako dijete udahne neko strane tijelo koje bi ga moglo života koštati.

Jedan od najvećih roditeljskih strahova je onaj da im dijete, dok je izvan dosega njihova vidna polja, ne udahne (proguta) neko strano tijelo koje bi moglo doći do dušnika ili bronha i na taj način proizvesti velike tegobe.
- Mala nepažnja može biti vrlo opasna, životno ugrožavajuća za dijete kojemu se nešto takvo dogodi - kazuje nam doc. dr. sc. Mirko Kontić, specijalist otorinolaringologije i subspecijalist plastične kirurgije glave i vrata sa Klinike za bolesti uha, grla i nosa s kirurgijom glave i vrata KBC-a Split.
Ako se nešto takvo dogodi, nad djetetom je potrebno obaviti ventilacijsku rigidnu traheobronhoskopiju (vađenje krutim tubusima stranih tijela iz donjih dišnih putova, dušnika ili bronha).
- Takvog pacijenta anesteziolog uspava i nakon toga mi pristupamo vađenju stranog tijela. Za to upotrebljavamo optičke hvataljke, uz čiju pomoć vidimo cijeli tijek zahvata i možemo ga “pod kontrolom oka” kontrolirati svaku sekundu i u slučaju poteškoća reagirati na vrijeme.
Pao na leđa i udahnuo ga
Ovi optički instrumenti (vrsta rigidnih bronhoskopa) umnogome su nam pomogli u radu, odnosno to je bila svojevrsna revolucija kada smo ih nabavili za bolnicu, govori dr. Kontić.
S obzirom na to da KBC Split ima optičke hvataljke, ovakvi zahvati koji spašavaju živote djece u pravilu ne traju duže od 15 do 20 minuta.
- Međutim, ima i izuzetaka. Tako smo nedavno imali zahvat na djetetu starom tek 19 mjeseci. Ono je dovedeno iz jednog grada srednje Dalmacije gdje je s roditeljima bilo na ljetovanju.

U jednom je trenutku dijete, tko zna kako, uzelo šest komadića kikirikija i stavilo ih u usta. Budući da je potom palo na leđa, udahnulo je taj kikiriki koji mu je ušao u bronhe. Taj zahvat je trajao sat i pol i bio je vrlo težak, jer što je dijete manje i sam zahvat je teži - pojašnjava dr. Kontić.
Roditelji često puta misle da su tegobe izazvane kikirikijima, bombonima... završene nakon što se dijete prestane gušiti ili kašljati.
Tri faze
- Postoje tri faze. Prva faza je kada se dijete guši ili kašljuca nakon što je strano tijelo ušlo u dušnik ili bronhe. Ta faza traje od 15 do 20 minuta i nakon toga prestaje.
Druga faza je faza smirivanja stanja, kada roditelji pomisle da se dijete samo “zagrcnulo”. Posljednja faza je faza komplikacija, kada strano tijelo ostaje u organizmu i od njega može nastati upalni proces, a na kraju može doći i do smrtnog ishoda - upozorava dr. Kontić.
U splitskom KBC-u ovaj postupak vađenja stranih tijela iz dušnika ili bronha obavlja se, prema riječima našeg sugovornika, već 40-ak godina.
Sve se prekida radi toga
- Obavljali su ga prim. dr. Duje Barač, dr. Mladen Puharić, dr. Šimunić i prof. dr. Ivo Glunčić, no oni su tada radili s puno primitivnijim sredstvima nego mi danas, kada imamo optičke hvataljke uz pomoć kojih vidimo cijeli proces vađenja.

Sada osim mene ove postupke radi cijeli tim od nekoliko ljudi s klinike i već educiramo i mlađe kolege koji će nas jednom naslijediti. Vrlo važan dio našeg tima su i anesteziolozi, anesteziološki tehničari i naše sestre instrumentarke.
Ovakvi zahvati su uvijek hitnoća i kada imamo neki takav zahvat, odmah prekidamo s operativnim zahvatom koji je u tijeku kako bismo mogli obaviti ventilacijsku rigidnu traheobronhoskopiju - govori dr. Kontić.
20-30 slučajeva godišnje
Godišnje se u splitskom KBC-u obavi između 20 i 30 ovakvih zahvata, a KBC Split za ove zahvate pokriva područje od Dubrovnika do Zadra i veliki dio susjedne BiH.
- Djeca najčešće aspiriraju (udahnu) kikiriki, lješnjak, badem, mrkvu, kukuruz, komadiće mesa, gumene bombone, kamenčiće s plaže, plastične ili metalne igračke, a nedavno smo jednom odraslijem djetetu izvadili kapicu od puntine.

No, taj je postupak obavljen za nekih 20-ak minuta, jer kao što sam istaknuo, lakše ga je raditi na većem nego na manjem djetetu - zaključio je doc. dr. sc. Mirko Kontić.
Stoga roditelji, dobro pazite i maksimalno brzo reagirajte ako dijete udahne neko strane tijelo koje bi ga moglo života koštati.
